Position Description

GHR Healthcare is seeking a qualified Nurse Practitioner to join a reputable organization in the Portland, OR metro area that provides Acute Care Services in the patient home.  This is a hospital at home care model that is on the forefront of modern healthcare. 

This would be a PRN contracted opportunity and very flexible (only need to be at least 3 visits a month)

This organization provides high quality, real-time nursing care and services along with all the necessary tools and infrastructure to safely shift advanced, acute medical care from hospitals into the comfort of the patients’ home. You will be conducting in-home patient visits.

Schedule:
•             As needed between the hours of 8am and 8pm
•             Visits scheduled based on Provider location and days of availability
•             Each visit could last between 1 and 2 hours in length. 
Additional Information:
•             Up to 4-5 visits a day
•             Must be Covid vaccinated with 1st booster and seasonal flu shot
•             3-4 Weeks of orientation/training

Responsibilities:
•             As the Nurse Practitioner, you will be responsible for providing high-quality patient care. You will serve as the point of contact for the patients and family in person, evaluating & assessing patients, collaborating with the attending physician in the organization and the patient’s primary care physician to create an acute plan of care in the home.
•             Performs the initial face-to-face evaluation and assessment of patients discharged from the hospital.  The NP will collaborate with the attending physician and the PCP to develop and implement an acute plan of care in the home.
•             Provide inpatient level acute care in the home setting in collaboration with the home hospital attending physician and RN
•             Communicate & provide education to the patient and family as well as assist them navigate the continuum of care


Salary:
•             $163 per visit

•             Medical / Dental / Vision + full benefits for anyone working at least 32 hours weekly

Qualifications:
•             Active NP license in the state of Oregon and eligible for license in the state of Washington
•             1-2+ years’ NP experience working within a hospital setting. 
•             Experience in internal medicine, acute care, hospitalist or ED is required.
