Position Description
Are you an APP looking to work in a high-energy practice?
Walking E Family Practice (in Twin Falls, Idaho) is seeking to add on a full-time APP.
Those local to Southern Idaho know that Twin Falls is a thriving community with a small city feel and a ton of outdoor access. Walking E is elated to serve our community with intentional medical care and we are looking for a phenomenal APP to join our team.
Key Responsibilities:
- Patient Assessment and Evaluation:
- Conduct thorough assessments of patient health status, including medical history, physical examination, and diagnostic tests.
- Formulate differential diagnoses based on assessment findings and diagnostic data.
- Develop comprehensive treatment plans in collaboration with physicians and other team members.
- Treatment and Management:
- Prescribe medications and therapies within the scope of practice and in accordance with established protocols.
- Perform procedures and interventions as necessary, ensuring patient safety and adherence to best practices.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Patient Education:
- Educate patients and families about their health conditions, treatment options, and preventive measures.
- Provide counseling on lifestyle modifications, medication adherence, and disease management strategies.
- Collaboration and Communication:
- Collaborate effectively with physicians, nurses, and other healthcare professionals to coordinate patient care.
- Communicate pertinent patient information, treatment plans, and updates to the healthcare team in a timely manner.
- Participate in interdisciplinary meetings and case conferences to discuss complex cases and develop coordinated care plans.
- Documentation and Record Keeping:
- Maintain accurate and up-to-date medical records, including patient histories, progress notes, and treatment plans.
- Ensure compliance with regulatory requirements and organizational policies regarding documentation and record keeping.
- Quality Improvement and Research:
- Participate in quality improvement initiatives aimed at enhancing patient care outcomes and safety.
- Stay current with evidence-based practice guidelines and contribute to research efforts within the healthcare organization.
Qualifications:
- Advanced degree (e.g., Master of Science in Nursing, Master of Physician Assistant Studies) from an accredited program.
- Current licensure and certification as a Nurse Practitioner (NP), Physician Assistant (PA), or other applicable advanced practice role in Idaho.
- Clinical experience in family practice preferred but not required.
- Strong interpersonal and communication skills.
- Ability to work effectively in a collaborative team environment.
- Commitment to ongoing professional development and evidence-based practice.
Working Conditions:
- This is a 4 day work week position in a fast paced family practice clinic.
